pdftoepdf.cc cleanup, needed xpdf-3.02 patching:
* /Metadata in /XForm is now indirect object (was direct/illegal before).
* keep indirect /Resources indirect.
* remove unnecessary initDictFromDict().
* copy objects by type once it's known.
* copy cases are now more uniform.
Currently no automatic /Group is generated by PDF in the enclosing /Page 
dictionary, hope that's ok. for now. No idea what to put into this /Group.
