Improve positioning of bottom accents

1) Always use top_accent position of the accents since bot_accent is not
   even part of the spec so always not set and the fallback centering
   will not work with the zero width accents. Eeven of the spec one day
   supports bot_accent, it make more sense to set it differently for the
   acntee not the accent.

2) Don't bother checking top_accent of the accentee as it will give bad
   result with italic glyphs and centering is a good compromise.
