Summary: Hyperlinks in included PDFs

Initial Comment:
Now when including pages from PDFs pdftex drops all
hyperlinks in the included pages. These should be retained.

keep in mind that this is non trivial because we can have
(1) namespace clashes and (2) more complex annotations than
simple hyperlinks, so we need quite some control over it (no
hacks -)
